Gone from unusual to painful?

My first bra that had cups had underwire and every bra since has. I can't feel it. Anyways I used to feel fine not wearing a bra at night but about 6 months ago it became unbearably painful. It hurt my nipples sooo badly. So I tried a sports bra at night and that was even more painful. Now I am back to underwire all the time only off for about 20-30 minutes all day for showers and changing into a new bra. Now (as of 4 or 5 weeks ago) it hurts to not wear a bra in the shower. Not just my nipples my whole chest hurts. Help? What could it be? (I can't ask my mom... Too embaressing)
